What to Bring

Be Prepared for Your Visit: What to Bring as a New Patient

To ensure we have everything we need to provide you with the best care, please bring the following items to your first appointment:

  1. Personal Identification
    A government-issued ID is required to verify your identity.
  2. Medicare Card or Private Health Insurance Details
    If you have a Medicare card or private health insurance, please bring these with you to help cover any eligible costs.
  3. Relevant Medical Records
    If you have past medical records or recent test results, bringing copies can assist your doctor in understanding your health history.
  4. Payment Method
    Our clinic accepts various payment methods if you need to pay out-of-pocket for any services. Bringing your preferred payment method will ensure a smooth checkout process.
