Transfer your patient records from SmartClinics Ipswich

Step 1: Contact your new clinic and request a transfer of medical records form.

Step 2: Complete the form and sign it.

Step 3: Your new clinic will need to send the request form to us via the below form

Step 4: We will liaise with your new clinic and facilitate the records transfer.
